DACO Membership Agreement

This form will be included in a membership packet that you will receive once your application and membership fee is received. You will neet to sign and return it to the Secretary as soon as possible so that there is no delay in processing your membership. Thank You.

As a member, I will:

1. Attempt to attend all meetings regularly and share my thoughts and ideas with the group.
2. Attempt to volunteer at least ten (10) hours to the group's activities in any way that I can. This will count towards "active member" status for 2006.
3. Welcome all who are interested in labor support, postpartum support, and birth.
4. Respect the decisions of the primary care providers that clients have chosen as described in the DONA Code of Ethics and Standards of Practice.
5. Sign a copy of DONA Internationl's Birth and/or Postpartum Code of Ethics and Standards of Practice according to my certification.
6. Follow the rules of the DACO organization as outlined in the By-laws and Constitution.
7. Seek approval before using the DACO Trademark and/or acting as a representative of the association.
8. Wear the official DACO Inc. polo shirt and act in a professional manner any time I am representing the DACO Inc. organization..
9. ALWAYS uplift my fellow doula in this organization and others alike.

In exchange, I can expect to receive all listed benefits under the membership information for one 12-month membership period. I understand and accept the above agreement as well as the intentions, concepts and ideals expressed in the Mission Statement of the Doula Association of Central Oklahoma.
